Investigation Summary
At approximately 10:30 a.m. on February 14, 2009, an employee was operating a Mitsubishi 5,000 lb capacity forklift and was moving empty citrus bins from the truck loading yard to the storage area. As the employee was reversing, the forklift struck a nearby empty trailer. His right forearm, which was in a crush hazard area of the forklift, was crushed in between the overhead guard post of the forklift and the LPG fuel tank that was struck by the trailer. The employee was taken to Community Regional Medical Center in Fresno, CA, where he was hospitalized and treated for a fractured arm.
