Employee Is Crushed Between Forklifts, Suffers Concussion

At approximately 11:37 a.m. on December 3, 2008, Employee #1 was using the Yale (standing operator position) order picker (Serial Number C801N07129E) in the warehouse. The order picker has very sensitive steering and Employee #1 lost control of the unit and collided with a forklift operated by a coworker. Due to the jolt of the collision, Employee #1's body recoiled and his head contacted the cages on both machines. Employee #1 was transported to the hospital. Employee #1 suffered a concussion and was hospitalized.
