Employee Lacerates Finger While Operating Machine

Employee #1 was operating a Peerless Plate Machine while forming approximately 9-in. diameter paper plates. The machine would operate at 32 strokes-per-minute with four lanes, and would produce approximately 128 plates-per-minute. The machine jammed on one of the forming dies, and Employee #1 attempted to clear the jam by reaching her arm into the jammed area from behind the die without utilizing any tools while the machine had not been locked out. While Employee #1's hand was in the machine, the power press initiated a revolution, cutting the operators left middle finger. Employee #1 suffered a laceration but was not hospitalized.
