Investigation Summary

Investigation Nr: 202505442
Event: 04/02/2009
Employee Fractures Arm While Cleaning Machine

At the time of the accident, Employee #1 was working on the sanitation shift, cleaning equipment. Employee #1 was instructed to clean a Paw Chiller that lacked machine guarding without being trained on how to deenergize the equipment. Employee #1 attempted to clean the equipment while it was still in operation. For an unknown reason, Employee #1 then reached down into the chiller, and the rotating bar trapped his arm between the arm and the side of the equipment. The bar then kept rotating, breaking Employee #1's left arm in three to four places. As the bar rotated past Employee #1's arm, he pulled clear of the machine. Employee #1 was hospitalized.
