Employee Amputates Leg While Cleaning Conveyor System

On October 15, 2009, an employee was cleaning a conveyor system that was not turned off or locked out. The employee got his foot caught in the conveyor. He was dragged into the conveyor which led to him to losing his leg above the knee. The employee was hospitalized and treated for leg amputation.
