Investigation Summary
On November 2, 2009, Employee #1 was working near a crane that was hoisting three roof opening frames. The rigging came off of the hook holding one of the frames and the frame fell. Employee #1 was struck by the falling frame and has injured. His injuries were not specified in the narrative. Employee #1 died on November 15, 2009.
