Investigation Summary
On October 26, 2009, Employee #1, a structural welder, was installing grates that measured 2 feet by 10 feet to construct a walking surface at level 253 in the SCR unit. While standing on an unsecured grate, Employee #1 fell between 24 to 27 feet and was killed. The accident investigation determined that Employee #1 was wearing a safety harness with two lanyards, but failed to attach the lanyards to an anchorage point.
