Investigation Summary

Investigation Nr: 200604254
Event: 06/10/2009
Three Employees Are Killed in Scaffold Collapse

On June 10, 2009, Employees #1, #2 and #3 were loading a mast scaffold with plaster from the thirteenth floor balcony of a 21-story multi-use building under construction. The mast scaffold elevator operator took the platform to the eleventh floor to plaster the wall. One side of the scaffold attached to the elevator by two bolts fell to the ground, striking the seventh floor outside parapet wall. Employee #1, #2 and #3 fell to the 7th floor deck to the ground, killing all three employees. A coworker on that side of the scaffold grabbed onto a balcony and was rescued. The other side of the scaffold remained in place attached to the other side of the elevator with additional workers unharmed.
