Employee's Fingers Are Amputated after Caught in Machine

On February 2, 2009, Employee #1 was operating a forklift, removing totes of plastic and aluminum containers from trailers at the dock area. The totes were then separated and placed on conveyors for processing. At approximately 2:00 p.m., when Employee #1 observed a tote stuck inside the bin unstacker of the "D" stacker conveyor line, he stopped the forklift and proceeded towards the full bin unstacker to fix the tote. Employee #1 opened the gate and entered the area behind the unstacker and then raised the folded door of the tote and placed his right hand on the top side of the tote. The unstacker unit then started and lowered onto the tote, crushing Employee #1's hand between the tote and the mechanical dogs of the un stacker. Employee #1 was able to free his hand and immediately departed the area. Employee #1 was taken to the hospital by emergency responders. Employee #1 sustained serious injuries to his right hand, eventually resulting in the loss of four fingers and half of his palm. The machine was not locked out, and Employee #1 was not trained in lockout/tagout.
