Investigation Summary
At approximately 12:00 p.m. on December 10, 2008, Employee #1 was trimming tree branches from the elevated bucket of an Anthony Tree Experts truck. According to a witness, the bucket was approximately 15 to 20 ft above the ground. Employee #1, who was operating the controls from inside the bucket, lowered himself down to connect a strap between the bucket and a medium-sized branch that had become entangled in the brush below after being cut. The witness reported that Employee #1 then attempted to lift the branch out of the brush by raising the bucket. After the boom momentarily bent, the strap failed. This caused the boom to suddenly jerk upwards and eject Employee #1, who landed on the ground, on his back, striking his head. When the witness went to assist Employee #1, he was unconscious, still wearing his hard hat, but was reportedly not utilizing fall protection equipment. He was transported, semi-conscious, to Shock Trauma in Baltimore, where he was admitted for treatment of a fractured skull.
