Investigation Summary
At approximately 9:15 a.m. on July 18, 2009, Employee #1, a salesman for a roofing contractor, and another employee, a claims representative for an insurance company, were on the roof of an existing residence to do an estimate for a roof replacement. When the task was completed, Employee #1 walked to the ladder, put his hand on the ladder side rail and his foot on the ladder rung. The top of the ladder slide away from Employee #1 along the gutter helmet and he fell approximately 18 feet to the concrete driveway. Employee #1 struck his head on the concrete driveway and was killed.
