Employee Is Killed When Crushed by Machine

At approximately 11:30 a.m. on February 2, 2009, Employee #1 was operating a hydraulic mop pressing machine. The machine had been down for maintenance the previous week and had not been reassembled. As the machine cycled the lower hydraulic ram, which had not been reinstalled, it tipped over and fell on Employee #1, striking his head. Employee #1 was killed. The machine had not been locked out or tagged as being out of service.
