Fatal Forklifts Collision

On November 5, 2008 at approximately 9:40 a.m., Employee #1 removed a pallet of deer corn with a Crown Reach Rider electric powered industrial truck an aisle away so he could place labels on it. Employee #1 then walked from the corn to get back on the powered industrial truck so he could position the powered truck to lift the pallet of deer corn and store it back on the storage rack. Employee #1 had the powered industrial truck in approximately a 40 degree angle toward Employee #2. Employee #1 did not look in the direction of his travel and collided with Reach Rider electric powered industrial truck operated by Employee #2. Employee #1 then steered his powered industrial truck forward and saw Employee #2 stand right off his powered industrial truck operator station. Employee #2 was transported by Greene County EMS Regional Hospital in Greeneville, TN where he was pronounced dead.
