Investigation Summary
At approximately 4:30 p.m. on December 8, 2008, Employee #1 was unloading the last of the vehicles from the car carrier when he slipped and fell approximately 8-ft to 10-ft to the ground level. Employees sustained a broken neck, head injury and a spinal injury. Employee #1 died at approximately 10:40 a.m. on January 4, 2009.
