Investigation Summary
At approximately 7:15 a.m. on May 6, 2009, Employee #1, a sanitation/production worker, was standing on a platform feeding product through the line. His left rubber glove became caught on the conveyor belt and his hand was pulled into the cutting knifes on the OV slicer machine. Employee #1 suffered an avulsion to the left index finger but was not hospitalized.
