Investigation Summary

At approximately 1:49 p.m. on February 25, 2009, an employee, a service manager at the automobile repair shop, went to retrieve a Vehicle identification Number, VIN, from a vehicle that was set on a lift rack, Hunter, as it was grounded. As the employee was writing the down the VIN number, the service coworker who was assigned to change tires on the vehicle lifted the hydraulic lift rack not knowing the employee was on the lift rack. The employee did not feel or hear the lift go up. When he finished retrieving the VIN number, he fell off of the lift rack and landed on his left heel. The employee sustained sprained ankle and bruised hip and back. Emergency medical services were immediately called and the employee was transported to Stanford Hospital, where he was treated for strains and sprains and released the same day.
