Investigation Summary
On February 16, 2009, Employee #1, a 45-year-old miner, with Buntich Pacific Jv, was moving concrete segments to a segment erector arm. Employee #1 moved into a position to observe and line up a concrete segment to be hoisted. As Employee #1 directed the operator to hoist the segment, Employee #1's right hand got caught in a pinch point. Employee #1 was hospitalized for a serious crushing injury to two fingers of the right hand.
