Investigation Summary
At approximately 9:45 a.m. on December 12, 2008, Employee #1 was observing a material handling machine he had made repairs to earlier that morning. A crane from the next aisle processed a cassette of steel rods onto the conveyor system while Employee #1 was bending down. The cassette struck Employee #1 in the head, killing him.
