Employee Is Killed When Struck by Falling Utility Pole

At approximately 7:45 a.m. on January 14, 2009, Employee #1 and Employee #2, of Sun Electric, were unloading approximately 53 ft of metal utility poles from a flatbed trailer. Employee #3 was operating the SkyTrack forklift used to off load the poles. Employees #1 and Employee #2 were working on the driver's side of the tractor/trailer. Employee #3 positioned the forklift next to the passenger side of the trailer. There were four poles on the trailer stacked two on top of two. Employee #3 began to lift the top pole on the passenger's side. The top pole on the driver's side rolled off the trailer, struck Employee #1 in the head, resulting in killing him.
