Investigation Summary
At approximately 8:50 a.m. on November 5, 2009, Employee #1 was working from a balcony that was being demolished. As Employee #1 was walking back to the stairwell, a section of balcony broke loose, and he fell to the ground. Apparently, Employee #1's back was fractured, and he was hospitalized. Employee #1 was not wearing a harness, and the balcony did not have a guardrails.
