Investigation Summary
At approximately 3:30 p.m. on November 20, 2008, Employee #1, a 46-year-old Korean male, was driving a forklift truck. The forklift struck a vertical I-beam support column, and apparently, the load backrest of the truck contacted the I beam. As a result of the contact, Employee #1 left his seat and struck his forehead, resulting in a skull fracture. Employee #1 returned to work on November 24, 25 and 26. He returned to the hospital on November 27 and died on November 29th. No one witnessed the accident, and it was, therefore, assumed that Employee #1 was not using his seat belt. There were no other extenuating factors that contributed to the accident. The driving surface was smooth concrete, and there were no surfaces that could have contributed to a tip over. During employee training, the company did not clarify rules requiring that seat belts be used at all times, when operating forklift trucks.
