Investigation Summary
On November 5, 2008 at approximately 9:00 am, Employee #1 was maintaining a forklift. Employee #1 had the forklift rear wheels removed and was repairing the rear axle. As he lay on the ground and had his left arm under the axle, the jack he was using, failed. The forklift dropped on his left hand. He sustained fractures and lacerations to his hand and arm.
