Employee Is Injured in Fall from Ladder on Scaffold

On July 14, 2009, Employee #1 was working from a 16 ft extension ladder that was set up on a 6 ft scaffold. He was approximately 14 ft up the ladder, nailing sheathing to the wall. He was injured when the scaffold tipped over and he fell to the concrete. Employee #1 was hospitalized for treatment of unspecified fractures.
