Investigation Summary
At Approximately 9:00 a.m. on December 10, 2011, Employee # 1 was working for his employer, Rain Gutters. Employee # 1 was standing on level three of the scaffold installing a downspout on the west building. Employee # 1 was checking to see if the metal installation was straight. As he checking he grabbed the end rail of the scaffold for support and the end rail gave way, causing Employee #1 to fell to ground. Employee # 1 was transported to San Francisco General Hospital where, he was treated for multiple fractures to his wrist and pelvic bone. The employer reported the accident to Cal/OSHA DOSH San Francisco District Office on December 13, 2011.
