Investigation Summary
Employee #1 was grinding to remove a layer of old paint off of a portable generator as part of their routine periodic maintenance. Employee #1 was operating a hand held, double insulated portable angle grinder with a wire brush attachment. As Employee #1 was using the grinder, it kicked back, and lacerated two of Employee #1's fingers.
