Investigation Summary
At approximately 9:30 a.m. on December 3, 2009, Employee #1, an attorney, arrived at the Law Offices of Benjamin, Weill and Mazer in San Francisco. He informed a coworker that he was feeling unwell and had so for several hours. Employee #1 went to the office of another coworker, an attorney and an emergency medical technician and informed the coworker about his condition. The coworker called emergency medical personnel, and reported Employee #1's vital signs and administered baby aspirin with approval of the 911 dispatcher. The paramedics arrived and transported him to Cal Pacific Medical Center. At the medical center, he received treatment and was hospitalized for observation. He was discharged three days later. From the treating physician, Employee #1's heart attack was attributed to diet and exercise rather than work stress. It was found that Employee #1's illness was predominantly not work related, and no accident- related violations were found after investigation. Other nonserious violations were found as a result of the investigation, and the employer was cited accordingly.
