Investigation Summary
On October 17, 2009, an employee, an event organizer and Salesman provided a tasting guest on the roof deck. The event was about over so the employee decided to clean the area of drinks. When he got up from the sofa, he moved to the right between the sofa and the coffee table, he tripped over guest feet and fell to the deck fracturing a right major upper arm. He was hospitalized and treated for his injuries and released on October 20, 2009.
