Investigation Summary
At approximately 4:08 p.m. on March 28, 2011, Employee #1 and a crew were installing 10 in. sewer piping in a trench. This trenching area had collapse before because of the previous location of the piping location, soil type and other unspecified factors. To protect employees, two shores with plywood backing were placed in front of a trench box (the end farthest away from the excavator) and one shoring device was placed behind (closest to the excavator). Each shore was 4 ft wide. As Employee #1 worked in the trench, it collapsed and he was struck by a large piece of earth on the back, neck and head. He sustained vertebral fractures. Several employees came to his aid. Emergency medical personnel were summoned. They arrived and transported him to a hospital. He was then transported by air ambulance to another medical center, where he was hospitalized.
