Investigation Summary
At approximately 10:00 a.m., on March 17, 2011 Employee #1 and Employee #2 were operating a hydraulic log splitter. Employee #1 put a log on the splitter while Employee #2 was operating a lever on the machine. Employee #1 was holding the log with his left hand and his left index finger got caught between the log and the edge of a metal beam underneath and amputated his finger up to the first knuckle and the fingertip fell into his glove. The employer transported Employee #1 to Tri-City Medical Center in Oceanside for treatment and surgery for about 6 hours and he was not in the hospital for an overnight stay. Employee #1 sustained bone and tissue loss of his left index finger.
