Investigation Summary
At approximately 2:00 p.m. on February 18, 2010, Employee #1 was moving the last piece of equipment, which was a press for making ball joints (25-ton Dura Press) weighing approximately 400 pounds. The press was about 28 inches wide and erected centered on two parallel metal legs, and stands 88 inches tall. Employee #1 tipped the press toward the wall and tried to walk the press out from underneath the storage racks on the edge of the presses legs. The machine fell away from Employee #1 and his right hand was placed at the top of the press to support it. Employee #1 suffered partial amputation of his right ring finger to the nail bed and pinky finger to the nail bed. Employee #1 was working for a company that fabricates aftermarket suspension parts.
