Investigation Summary
At approximately 8:33 on November 5, 2009, Employee #1, an equipment handling supervisor, was standing on the ground, removing metal shoring from the back of their rental truck at a road construction site. He caught his left index finger in-between the hinge of the speed shore hydraulic spreaders and the movement of the metal shoring, causing it to be amputated. Employee #1 was transported by ambulance from the construction site to Palomar Hospital. He was not hospitalized for his injury.
