Employee Is Injured in Fall from Ladder

At approximately 8:30 a.m. on June 18, 2009, an employee had climbed the ladder to retrieve a pair of tin-snips he had left on some ductwork that he was working on during the previous day. The employee had accessed the A-Frame ladder and had climbed to the fifth or sixth step and was reaching past his center of balance while blindly feeling for the tin-snips that were overhead in the ductwork. When the ladder shifted, the employee fell to the floor and struck his head on the floor causing a severe concussion. The employee was hospitalized and treated for concussion.
