Investigation Summary
At approximately 10:00 a.m. on April 27, 2009, Employee #1, a project manager, was working at a childcare center in Escondido, CA. He was asked to repair a retracting sunshade in the playground area. Employee #1 used a six-foot fiberglass stepladder to inspect the electrical box for loose fittings. He set the ladder up in the sand at the base of the sunshade. Employee #1 was on the fourth step up, with one hand on the box and one hand on the ladder. The ladder tipped to the right and fell over. As he fell, his right leg got caught in the lower rungs, breaking the bones and twisting his right knee. He was transported to a medical center, where he received treatment and was hospitalized.
