Investigation Summary
On October 8, 2008, an employee and the logging crew were assigned to log approximately 20 acres of land. The crew assigned to the section where the accident occurred consisted of one tree cutter operator and the employee and a coworker, two chain saw operators. The cutter operator cut down trees while the chainsaw operators trimmed the branches from the trees until the skidder operator could remove them from the section. The cutter operator was cutting down trees and did not see or hear the two chainsaw operators. Vegetation obstructed the view between the cutter operator and chain saw operators. The employee and the coworker were topping trees approximately 50 feet from tree cutting operation, when a tree fell on the employee. The coworker was uninjured. At approximately 3:05 pm the cutter operator phoned emergency medical services, while the coworker attended the employee. At approximately 3:20 p.m., the Wachepregue Melfa Fire and Rescue Squad arrived and pronounced the employee dead. The employee was then transferred to Cooper and Humbles Funeral for holding. The employee was a part time employee with no documented training. No personal protective equipment was worn by the employee at the time of the accident.
