Investigation Summary
On October, 5, 2008, an employee was operating a sheet folding machine in a laundry service. He did not turn off the rollers before climbing on a shelf attached to the front of the machine to remove a pillow case that was stuck in the rollers. While getting down off the shelf, she slipped and her hand went under the guard and became stuck in the rollers. She received bruised fingers. The employee was not hospitalized.
