Investigation Summary
At approximately 2:45 p.m. on February 28, 2012, Employee #1 was helping with the trimming of a tree. The employee was working off a roof top that was approximately 24 ft. high, without any fall protection. Employee #1's work included replacing carpet, replacing old cabinets, repairing the stairs, and painting vacant apartments. On the date of the accident, Employee #1, 2, and 3 were trimming a tree from the ground using a rope. Employee #1 and 2 went up approximately 24 ft. to the roof to cut the tree branch. Employee #2 was using a gas powered chainsaw to cut the branch that was on top of the roof into pieces, and Employee #1 was throwing the cut branches down to the ground. Employee #2 and 3 were throwing the cut branches that were on the ground into the employer's truck. Employee #3 cut the last piece of the branch while Employee #1 was holding it approximately 3 ft. from the edge of the roof. Employee #1 noticed the branch turned, so he let go of the branch. When the branch came loose from the tree, it turned striking Employee #1, and pulling him to the edge of the roof. Employee #1 tried to balance himself to remain on the roof but could not hold on. The employee fell approximately 24 ft. to the ground. There were no guardrails at the edges of the roof. Employee #1 landed standing on his feet, resulting in fractures to both ankles. The employee was hospitalized for more than 24 hours for treatment
