Investigation Summary
At approximately 12:10 p.m. on January 12, 2012, Employee #1 was cutting a metal plate on a shear press, and when he tried to pull the plate away the press cycled and plate fell on his left index finger resulting in a crushing injury to the finger. Employee #1 was taken to a local hospital for approximately 4 hours, where his fingertip was amputated due to the accident.
