Investigation Summary
At approximately 10:15 a.m. on November 1, 2012, Employee #1, was crushed by a pound coil being unloaded from a flatbed trailer. Employee #1 proceeded to climb onto the flatbed trailer to remove chains and straps from the coils at the rear of the trailer bed being unloaded. Employee #1 was standing between the steel coils and the side edge of the trailer, and cut the bands from one location. Upon doing this, Employee #1 was struck in the head and right leg by one coil that was displaced and knocked off the trailer to the ground below. Employee #1's injuries included an amputation of the right leg, above the knee. Employee #1 was hospitalized to recover from his injuries.
