Employee's Fingers Are Cut When Caught by Dough Rollers

At approximately 11:14 a.m. on August 10, 2011, Employee #1, a temporary production worker with three years' experience, was working with the automatic dough rolling machine (CHAN MAG machine, Model Number FM 320, Serial Number HM 5054 F). As he was pushing dough through the machine, his gloved left hand got caught in the dough going into the rollers. He was able to stop the machine by pressing the emergency stop with his right hand. After he was freed from the machine by his supervisor, he was taken to a nearby hospital, where he was treated for severe lacerations of his left index finger (0.8 inches), middle finger (1.2 inches), and tendon sheath involvement of his middle finger. He was not hospitalized for his injuries. It was determined that the causes of the accident were: failure to properly train employees; failure to enforce the use of the guard on the machine, and a defective sensor.
