Investigation Summary

Investigation Nr: 202477279
Event: 02/11/2011
Employee Fractures Hand in Machine

At approximately 8:45 a.m. on February 11, 2011, an employee was working as a packer, putting filled bottles of Horchata into boxes. The employee walked to the area of the machine where bottles were being filled to check for jamming. He slipped on the floor. He attempted to catch his fall, but his right hand went into the area where platforms containing empty bottles are raised to be filled with Horchata. The employee's right hand became stuck in the machine, and he yelled at a coworker to stop the machine. The maintenance department was summoned to remove machine parts to free the employee. The employee was transported by paramedics to Kaiser Permanente in Baldwin Park, California where he was treated for two fractures on the right hand. He was released at 7:00 p.m. the same day. The employee was wearing rubber boots during the accident. The identical rubber boots were tested for friction against the floor area where the employee slipped and found to be non-slippery. There were no hazardous nip points in the machine area where the employee's right hand was caught. The employee was unsure where exactly in the machine his hand was caught. The employee had been operating the machine for five years prior to the accident.
