Investigation Summary
On December 9, 2008, Employee #1, a mechanic, was carrying 40 pound buckets of cream cheese into a refrigerator. Employee #1's responsibilities were to maintain the fleet vehicles for Los Altos Food Products. Employee #1 was assigned to do another task of stacking 40 pound buckets of sour cream in a refrigerator. He felt pain in his lower back and reported it to his supervisor. While receiving medical treatment, Employee #1 continued to work until August 2009, when he could no longer tolerate the pain. On September 1, 2009, Employee #1 had back surgery. He was hospitalized until September 4, 2009.
