Investigation Summary
At approximately 12:50 P.M. on May 12, 2010, Employee #1 was conducting routine cleaning of print plates on the inside of a McKinley Rotary Die Cutter Printing Press Machine. A coworker, using a Toyota Forklift pushed the printing press machine closed from the feed side and crushed Employee #1 while he was inside the machine. Paramedics arrived at the work site and Employee #1 was taken to the Hospital, shortly thereafter, Employee #1 was pronounced deceased. Employee #1 was inside the printing section of the McKinley Rotary Die Cutter Machine without the machine being mechanically blocked to prevent movement.
