Employee Amputates Fingers in Flow Control Valve

At approximately 8:30 a.m. March 27, 2010, Employee #1 was changing the gaskets on valves in the pasteurizing area at Alta Dena Dairy. Employee #1 and a coworker were assigned the task of changing the gaskets on a Top-Flo Automated Flow Control Valve (Model Number 7100036AT), a new task responsibility for operators in the pasteurizing area. To remove the Top-Flo from the system, Employee #1 and the coworker had to apply a pressurized air line to close the valve's stem. After removing the clamps and then removing the Top-Flo valve from the system and placing it on the floor, the air line was removed. This caused the valve stem to reopen. Employee #1 and the coworker replaced the gaskets. Before putting the Top-Flo back on the system, Employee #1 and a coworker had to hook the air line to the valve to close the stem. Employee #1 was holding the valve in his left hand, while the coworker was connecting the air line. The applied air caused the stem valve to close on the fourth and fifth fingers on Employee #1's left hand. Paramedics were called and Employee #1 was taken to Queen of the Valley Hospital in West Covina, California and admitted. Employee #1 was released the next day. The following Wednesday the doctor amputated both fingers. On March 27, 2010, at approximately 9:30 a.m., West Covina District Office received a report of the accident. It was determined that Employee #1 had been assigned a new task without the employer, Alta Dena Dairy of City of Industry, California, identifying unsafe conditions associated with that task.
