Investigation Summary
At approximately 3:30 p.m. on September 4, 2009, Employee #1 was working with a coworker retrieving boxes of produce from the top of the storage rack at Super H Mart, a grocery store. The storage rack was 95 inches high, and utilized cross beams and a wire mesh grate to sustain the weight of the boxes and employees that would retrieve them. The wire mesh grate had rectangular openings of approximately 2 inches by 5 inches. E1 was standing on the storage rack's wire meshing and handing boxes of produce to a coworker on the floor below. The coworker then placed the boxes on a pallet on the ground. After the boxes were placed on the pallet, the coworker began to pull the pallet towards the entrance/exit of the cooler. At the same time, Employee #1 was arranging boxes of produce against the wall on the storage rack. Employee #1 lost his balance while stepping on the wire mesh grate and fell 95 inches from a storage rack to the floor in the refrigerated produce storage area. E1 sustained a chipped tooth, trauma to his chest and wrist sprains. The Division was notified on October 20, 2009, by the General and Store Manager. Employee #1 was working as a Deputy Chief of Produce with Super H Mart in California.
