Investigation Summary
At approximately 1:20 p.m. on June 30, 2009, a maintenance worker was employed at a plastic injection molding facility. The worker was hosing down the rooftop of the building to remove accumulations of dirt. As the worker was hosing down the roof, he tripped on the edge of a skylight and fell through it. The skylight was about 20 ft above the ground. The skylight opening measured about 52 in. by 100 in. with a toe board about 4 in. high. The skylight cover was made of plastic. The worker was hospitalized for the treatment of fractured ribs and a punctured lung.
