Investigation Summary
At approximately 10:15 a.m. on May 8, 2009, Employee #1 was working at the jobsite along with a coworker on their last day of a three-day job. The equipment involved in the accident is a 20-ft aluminum extension ladder. Employee #1 attempted to climb the ladder to the roof to install solar ventilation. Employee #1 fell approximately 9-ft 6-in. to the concrete ground just as he stepped onto the roof's eaves. Employee #1 hit the right side of his body to the concrete surface. The extension ladder was not secured at the top or bottom to prevent its movement while being use. According to a coworker, the feet of the ladder were bare aluminum and missing the rubber pads. Employee #1 suffered a fractured rib and pelvis, and was transported by ambulance to the hospital where he was hospitalized.
