Employee Partially Amputates Finger on Grinding Machine

At approximately 3:00 p.m. on March 5, 2009, Employee #1, a machine operator, was grinding a 2-inch by 0.625-inch metal fastener on a centerless grinder for a set up. As he proceeded, the piece got stuck in the grinder. He tried to remove the piece with his left hand without deenergizing the grinding machine, and his index finger got pinched in the moving parts, resulting in a partial amputation. He was taken to a nearby medical facility where he was treated, and released after four hours on the same day. Employee #1 stated that he was trained in Lockout / Tagout safety rules by his employer, and should have used a hand stick to dislodge the stuck metal piece from the machine, as required by company safety policy.
