Investigation Summary
At approximately 2:45 p.m. on October 28, 2008, Employee #1 was digging a trench at an established residence for the purpose of upgrading a water line. At a depth of approximately 6 ft, the vertical side wall of the hand-dug trench collapsed and the spoil pile fell onto Employee #1, burying him. Employee #1 was killed.
