Investigation Summary
On September 25, 2008, three employees were working as part of a road signing construction crew. The employees, who were occupants of Vehicles #1, #2, and #3, were standing in the rear of the trailer of Vehicle #1, a truck with a traffic control device in the back. Vehicle #1 was parked in the right-hand lane and was moving traffic into the left-hand lane. Another truck, Vehicle #2, was parked in front of Vehicle #1. Vehicle #3, a truck with a double axel flatbed trailer that contained traffic cones, was parked in front of Vehicle #2. A semi truck apparently struck the rear of Vehicle #3, resulting in a chain reaction in which Vehicle #3 struck Vehicle #2 and Vehicle #2 struck Vehicle #1. All vehicles and trailers came to rest on the southbound shoulder and in the west ditch of the southbound lanes. Two construction crew employees were killed. One construction crew employee and the driver of the semi-truck were hospitalized for unspecified injuries.
