Employee Is Killed When Trapped Under Draw Bridge

On October 8, 2008, an employee, a conveyor operator, was lowering the drawbridge, when one of the drawbridge sections failed to lower. Because two position valves were used to control the air pressure cylinders used to raise and lower the drawbridge, the section that remained elevated was continuously under pressure. The employee left the control station, releasing the drawbridge control switch and headed to the drawbridge. During this time, a latch assembly stop broke at the weld, releasing the drawbridge section to lower to its down position, trapping the employee. The employee was killed.
